In December 2020, Dr. Sara Panella, Manager, Transitions of Care at Baptist Health South Florida, contact Cheri Lattimer, Executive Director of NTOCC, regarding a challenge she faced leading a team of transition of care pharmacists, as well as the overarching dilemma of an organizational source of professional advocacy for the role of pharmacists in transitions of care.

This led to the development of a 15-member multidisciplinary Pharmacy Transitions of Care (TOC) Advisory Task Force, co-chaired by Sara Panella and THCQ Consulting, Inc. President and CEO, Norris Turner.

After 5, virtual, lively, 1.5-hour meetings over 5 months, the Task Force achieved its objectives and most notably, developed the Ten Principles of Pharmacists in TOC.

A permanent NTOCC Pharmacists in TOC Committee has been newly convened where four subcommittees are focused on advancing the field in the areas of:

(1) Promotion of the principles;
(2) Creation of patient-facing materials;
(3) Value-based care/payment for pharmacists in TOC; and
(4) Community pharmacy engagement in TOC services.
